In 2001, the National Academy of Sciences Institute of Medicine (IOM) laid out six dimensions of quality for health care. According to the IOM, care should be safe, effective, patient-centered, timely, efficient, and equitable.

Carol Haraden, a Vice President at IHI and patient safety expert, tells us why safety is at the top of that list.
