no. It doesn't hurt. It's the same as putting a band aid on yourself. The purpose is for the ears to stand erect on the adult dog, thus creating a look of glamour and elegance.

the basic concept is the same regardless of the breed or the ear length. In smaller toy breeds, you would have to get very small foam that is super light....you don't want the weight of the support to be too heavy for the ears. In some toys breeds that have naturally erect ears (not cropped) you can also assist them in getting up by simply taping and not using the foam support.
