You have to buy the hardware that goes with this app. It's an obd-II -> WLAN device. The iPhone hooks up to this wirelessly via WIFI. (the device creates an ad-hoc network)

It is compatible with any car that has an obd-II port. Basically any car after ~1996 has an obd-II port
