at 24s a tool comes down and spins - is it supposed to be screwing in the last part? Just that this part is already fully in and doesn't turn round so I'm not clear what is going on.

It looks a bit painful and dangerous but you don't feel anything because all the procedure will be after doing numb your mouth and after the procedure, it will get adjust into the mouth but to you need to take all the precautions as suggested for smooth adjustment.
