On a night with light to moderate traffic at Los Angeles International Airport in 1991, an air traffic controller mistakenly assigns an inbound USAir Boeing 737 the identical runway where a SkyWest Airlines commuter flight is awaiting departure clearance. Upon landing, the 737 collides with the small aircraft, skids off the runway, and strikes a building. All 12 passengers and crew on the SkyWest flight are killed, along with an additional 22 occupants on the USAir flight.

Light to moderate ATC? It was packed full with aircraft. It was heavy traffic which is why they forgot about the Metroliner.
