Neptune in it's higher octave is the spiritual, in it's lower state you have the drugs, alcolhol, confusion, deception, deceived. An entity following the higher octave would use a difficult aspect for growth. Neptune square Mars also can relate to the "gay movement" which we see active in the 60's and increased now, under the similar aspect. So much is there, but interpreted by the viewer and their own nativity.
