My quick review of this mishap : terrible CG placement, rotation happens at very low speed with so little up elevator (approx 5 degrees), then you can see the pitch instability and the 'dolphin kick' in pitch with obvious PIO, finally it looks like the left flap got stuck full down,creating both a roll to the right and killing airspeed and then entering an unrecoverable spin.Always good to place a ton of lead in the nose for 1st flight. The whole thing being worsened with thrust line below CG...

I beg to disagree. I think he did a smashing job of getting the nose to come down.
