Incorrectly loaded umbilical !! With devastating consequences

If you notice they have incorrectly loaded the umbilical product and it leaves the reel from the top of the spool. They have obviously lost tension on the load, with no break system in place to stop this run off.

A lot of lessons to learn here with one glaringly mistake of not loading the products correctly i.e. it should leave the reel from the bottom and this give them more control & not over load the tensioner ..

Great vid! Brings back alot of memories.

What had happened was the "chinese finger" which was used to hold the weight of the pipe dangling in the water had let go. The brake was in fact on to try to keep the reel from free spinning off of the under roller system. Once it started though, there was no stopping it.

Amazingly, not one person onboard the Ocean Intervention during this was injured.

at the start it looked like a brake was stuck on but i may be wrong just seen it heaps in race cars and i am guessing the would have a break on the drum

someone either forgot to grease the thrust bearing or something was pulled in and grinded against the reel housing until it seized the motor or caused it to burn out.
