they never said it was an "accident". it was a mistake, mistakes are deliberate, such as "it was a mistake to cheat on my wife. i knew it was wrong but i still did it.". accidents imply that no one is responsible. which is why they used the word "mistake" their PR person at least knows proper grammar. they are owning up to the bullsh!t and that they are douchers. however, by saying, "people throughout the company have been spoken to..." that means nothing was done and no one was reprimanded.
