This is an example of the tennis elbow test. It is a very effective means for identifying problems with chronic tendon degeneration of the common extensor tendons at the lateral epicondyle.
I had ruined elbows for 8 months (triceps tendonitis at elbow insertion, posterior impingement syndrome of the olecranon, golfers elbow AND tennis elbow, on both elbows!!). Almost got a bunch of surgeries until I found a great prolotherapy doctor in Illinois (go to Dr. Ross Hauser if you can!!).
Prolo is the opposite of cortisone injections. Prolo uses irritating solutions to make inflammation in the area, re-instigatin healing
Having tennis elbow for that long has probably caused pathologic changes to the muscles in your forearms. conservative treatments probably wont work quite as well for you.
I highly recommend looking into prolotherapy. Google it. Read up. It's much cheaper than surgery, and thats what a doctor will advise you to do since youve had the condition for so long and your elbows have been healing all messed up (inevitable with tennis elbow for 10 years)

Platelet rich plasma injections are also good.
