Here is exactly what happened: Kurt tries to crowd surf. As the bouncer tries to attempt to guide Kurt back onto stage, he grabs Kurt. Kurt misreads this as a hostile action, and begins to kick and hit the bouncer. The bouncer, enraged that he is being hurt for trying to help, whips Kurt onto stage and begins to try to beat him out of frustration.

Actually, according to Heavier then Heaven, Kurt kicks amplifier, amplifier belongs to bouncer, bouncer gets mad, kurt jumps into crowd, bouncer gets revenge, kurt slams guitar into bouncers head. Thats what set it off.
