I think they knew exactly what they were doing, they wanted to make the ending shocking. I mean few people would expect a show that starts off all cute and romantic to turn dark and disturbing at the last minute.

We never know for sure if she was pregnant. Even though Sekai cut her womb open, she couldn't have been more than a few weeks; the embryo would be invisible to the eye.

Oh, and yeah; Kotonoha basically went batshit, which is why she kept the head.
