at the first time i played it scared the shit out of me then i dared myself to play this level alone at the dark i almost shit to death while playing this level

This level was great - but it's even greater if you turn off the rock music in settings and let the creepy atmospheric sounds continue during the fights.

This level isn't creepy. I got past it using only melee in the dark, with the sound.

The scariest level for me in Painkiller,I didnt expect to see so many creeps in the asylum when I first played! X+

@chrysanthos66 Not anymore, since 2009, it was demolished because of safety issues, but yeah, that building was Danvers Insane Asylum in Danvers, Massachusetts.

The environment and that feel of an insane asylum when you look at the enemies can be creepy but it's definitely one of the most fun levels in the game =D I always get a good kick goofing around with my stakes and those crawling patience xD Piercing their tongues was always fun as hell.
