It's Iommi alright, jum to 3:54, there's a closeup of his left hand on the guitar's fretboard. You can clearly see the solder/metal caps he puts on his finger stumps he uses to play, that can't be anyone else but Tommy!

Ok, it looks like Ward on drums and Butler on bass, but I have NEVER seen Iommi without a mustache so I am guessing that is what is throwing me off, also the fact that Iommi is usually kind of laid back and to Ozzy's left just a bit and this guy in this video is all over the place, plus I can't find the cross Iommi is famous for ALWAYS wearing, but these were also the cocaine days so that could be where his energy is coming from. I'm fairly certain that they were all there that day. I haven't read nor heard much about 'stand-ins' or replacements.
