Best to think of tengo que/tienes que as set phrases meaning I/You have to or I/You must. You don't usually use the que to make 'to' e.g. I want to see = Quiero ver . No que is used.

You just need to think of 'tengo que' as a phrase meaning I must or I have (to). It works out as I have that to speak, if we translate literally.
