There was an episode of "Pitchmen" where they showed the making of this commercial, and Billy was too scared to put his hand in harm's way, so they had to have a stuntman do it.

it's the sudden impact from the car driving over your hand that is rippled, and it stops before it can equalize and actually weigh down on you. Also, not sure how it would be faked.
