This is a 70 year old engineer with a decentered Tecnis IOL within the capsular bag. He had dysphotopsia due to the exposed inferior edge of the lens and maddox rod type glare from adhesions between the anterior and posterior capsule causing folds in the capsule. He was also +1.0 hyperopic. He has Fuchs dystrophy with 2++guttata and an 1100cell count. I decided to collar button capture the new lens with the optic in the bag and the haptics in the sulcus because I was unsure whether the posterior capsule was completely intact. The next day his cornea was clear and his dysphotopsia symtoms completely resolved.
