that's because Leona & Mai got double KO and in those cases both teams lose a member, since the first team only played with 2 characters, the third charatcer (Iori) appears in the next round, but the other team has the 3 members defeated, so Iori wins even if he did nothing

if the two teams make double KO with the third member, it will be game over

From what i understand though,

the core gameplay is basically like the old ones ('98, 2k2 etc.) but with extra mechanics to mix it up.

I heard the game has four glitches that have been made known because of the Tougeki tournament. Three of them are bannable. They will patch them, but not in time for Tougeki.
