@Moscovo The "adequacy" of the boat is not the issue. The job is inherently risky & dangerous. This accident took place in January of 2006. That same year 4 other American pilots were killed during transfers. Another pilot boat captain was killed when his boat rolled over while alongside a ship in rough weather and was trapped inside while it sunk. As pilots, this type of accident is the one you worry about all the time and pray never happens, to anyone.

While transitioning from the Island Princess cruise ship to the waiting pilot boat, Capt. David "Kawika" Lyman was pitched overboard, injured by the pilot boat, possibly even the propeller.
He was recovered by the pilot boat captain, and brought to Nawiliwili Small Boat Harbor, where members of an awaiting American Medical Response ambulance crew brought him to Wilcox Memorial Hospital, where he was pronounced dead.

I've searched in the internet for the key words "Nawiliwili harbour pilot accident"
Since the pilot boat (named Ninini) is registered at that port in Hawain Islands.
In the results it appears an accident where the pilot dies from its injures. So unfortenly RIP.
This is a risk job. But in this case I think there is negligence from the part of the harbour authorities. Since this pilot boat is not adequate for this specific operation with tall side ships.

@st3wart69er Why the fuck would it be a shark attack? Do you think sharks attack every possible thing that falls in the water? Shark attacks are more rare than being hit by lightening. Look it up. When someone is killed outside do you automatically assume he was struck by lightening? Im so sick of people being so afraid of the fucking ocean. Go swimming. You wont die.

probably he was injured by the impact... he might felt on the pilot vessel... their reaction is good.. keep the vessel between the pilot and the cargo vessel... hope he is well... but there a lot of incidents with pilots and pilot ladders...
