It's really creepy how just after I listened to this, I found out my friend's relative just passed away...

A breath that is not your own sounds in the darkness. You look up sharply, and there it is again. Reluctantly, you set down the book and creep out of the study and down the hall, towards a room at the end. The breaths grow louder, shallower... a woman is dying. Madeline... somehow you feel her name across the space between you. Like a child, you tiptoe closer to the door and turn the knob. A sudden, forceful breath is exhaled into your ear, like a cruel prank, and then the presence is gone.

I imagine that the main character comes into a bedroom where he sees someone lying in bed. He can't see who it is because there's a blanket draped over the person. At the last breath the hero pulls away the blanket there's no one there. (That's why the last breath sounds so surprised)
