Was probably a vortex ring state. Given the bad weather the pilot was probably descending to fast vertically.

Damn, if they hadn't have lost the tail rotor he probably would have been able to correct that

look closely at the pilot door at 0:10 it looks like the gets open and the pilot falls out

The enquiry said it was rotor droop.

Aviation Week dot com has a small write up that indicated it was either a GPS problem or "rotor droop", a condition where the downwash of the rotor was at the same speed as the descent of the copter, causing the blades to provide no lift.
