@baixiaolang Not really, he's always appeared as soon as Caves 1 for me, even when I first got the game from the Voxatron bundle (Nov. 1). So far I have had unlocked the Everything's Awful ending, so now the game is harder for me.

The Fallen is actually an enemy/boss that was added in the Halloween update, but I've never seen him appear outside of a specific level (later than any in this video). I guess that was added in a recent update?
