modern helicopters may be reliable, but they sure aren't when they been hit by an anti-air missile...
anyway, everyone makes mistakes, so it is not always just the aircraft's fault.

@mckseal in cold war times there was a prototype in US based in ejection in phases
phase 1 a smal amount of explosive detonates near the axis of propellors
phase 2 inertial force or centripete force throws away propellors
phase 3 roof "leaves" hull
phase 4 seat and pilot ejection
but actually no US helicopter have that system because never i think was dangerous and not flawless at all
anyway modern helicopters are so reliable

you know why the russians are smart? they built EJECTION SEATS into their helicopters :). so they survive a crash.

if you are fascinated by it, i recommend you get DCS:BLACKSHARK.
its a simulation that models this helicopter, and a good one at that. just be patient, its not easy :)
i'm fascinated with the ka-50 and have been for a long time how fast is it? what's the payload?...i'm fascinated also with the dual rotor stacks the negative torque choppers of russia not to be confused with our chinook
