Amazing song, and it's about a common thing that happens in Thailand. Girls getting forced by their parents to be with a guy who ears lots of money (in this case a doctor) then a guy who she really wants to be with (in this case a fisher)
In the end, the fisher saves the doctor from drowning, giving up his life for him.
Try getting such a story in music these days :o
