It's 30 hours before Dean's deal with the crossroads demon is up and Dean, Sam and Bobby are searching for Lilith, who holds the contract. They discover her location and Sam summons Ruby for help despite Dean's protests. Ruby warns them they aren't ready to fight Lilith yet but they steal her knife and depart for New Harmony, Indiana.
In a battle to the death, Sam, Dean and Bobby take on Lilith and all her demons in a last ditch effort to save Dean's life.

you can see a woman looks like the way Sam does this moment...and she didn´t have anything to do with supernatural or demon blood

there's a switch role in this. sam is more like a big brother when he's trying to comfort dean.

If you watch the episode, Dean, apparently starts to be able to see demons in their true form. As he gets closer and closer to dying, he can look at demons and he doesn't see people, he sees their true fugly faces...I think what he saw when he looked at Sam in this scene is a glimpse of Sam's true form. Sam has demon blood in him, and if he embraces his 'dark side' that is what he would truly look like. I think its a great scene that forshadows what is going to happen to Sam next season.
