This is a patient referred for a dislocated IOL and astigmatism after scleral buckle and PPV for retina detachment. The capsule is adherent to the iris temporally and she has a zonule dialysis in this region. The IOL is in the collapsed bag and decentered. First the adhesions between the capsule and the iris are lysed using the tip of 30gu needle with traction from a microforceps and then a scleral lasso suture is performed. After scleral fixation the IOL is well centered and has remained so.
