Probably better that it was a bullseye decapitation rather than the blades partially cutting them and/or crushing them and suffering a slow painful death.

No it wasn't the pilots fault. It was Director John Landis fault. The pilot was told to fly 20ft above the village with pyrotechnics going off all around him. One of the explosions blew off the tail blades and when that happens they say the pilot becomes a passenger. Read the story behind this and you will realize Landis was warned by the pilot and the helicopter staff that this was unsafe at even higher elevations. Back then Landis took chances to get things out ahead of deadlines.
