This trip goes:

1) Take a tuk-tuk from Betong, Thailand to the checkpoint, fare is 200 baht (US$6.50). Pay 20 baht (US$0.65) overtime fee to the officer. This is completely legal at this checkpoint - there is no corruption involved here.
2) Walk to the Malaysian Immigration, fill up a form and have your passport stamped, no fees required. Note that some countries need a visa prior to arrival.
3) Take a taxi to the bus terminal at the town of Baling, a 30 minute ride from the border. Enjoy the scenic landscape on the way. Fare is fixed at RM 25 (US$8.00).
4) Take a bus from Baling bus terminal to Butterworth, Penang. This is a two hour ride which cost RM 7 (US$2.40).
5) Upon arrival at the Butterworth bus terminal, walk to the ferry terminal and take a ferry to Penang Island. The ferry cost RM 2 (US$0.65).

In case you want to go to Singapore:

From the Butterworth bus terminal, take a coach to Johor Bahru (JB). It is usually an overnight trip to JB that costs RM 65 (US$20.80). From Larkin Bus station in JB, you can take a bus to Singapore. It is less than one hour ride for a fare of RM 2 (US$0.65)
