If the liver appears in black, why does a baby ultrasound appear in gray? And can't we do a 3-D ultrasound of an organ?

At about 4:20 you inadvertently happen to misname the hepatic veins "portal veins". Apart from that it's a good demonstration of basic liver anatomy for ultrasound.
