ehh it depends...the lowest line is usually the cable tv system...sometimes a combination of phone and cabletv...the middle lines are usually stepped down voltage (to houses and small businesses) and the highest up is usually the 4-17,000v lines...sometimes there is another set even higher than that which is 100,000+ volts

Yep, type of fuse was a 'drop out fuse'. Generally when they blow and no fault is found they attempt a reclose on the line. Since this blew pretty quick it is a major fault. Could be a wire down, blown transformer or a blown lightning arrestor. The less common ones are people throwing stuff over the lines. ;)
