Put it here:
C:\Program Files\Steam\steamapps\common\left 4 dead\left4dead\cfg
To start it open the developer console (~) and type:
exec horror
To disable it just restart the game, set your controls back to default and everything should be back to normal.
Additions to the controls:
Shift: Sprint (Burst)
Alt: Camera Change (1st/3rd)
E: Revive Self frrom Incap and Hanging
Reccomended on:
Singleplayer
Easy for good times.
Normal for challenging but fair.
Advanced might be possible.
Expert is impossible though.
Only downside is you have to manually reload the campaign, death check is off so you'll just lie there for eternity until you do.
Many Player and Zombie changes have been made to add combat options and make the singleplayer experience actually fun instead of a totally pointless side thing. I've off and on spent the better part of a year balancing this mod until I was able to hit a "sweet spot" where it was both fun and challenging, without being frustrating. Please check it out and tell me what you think.

Also note that this video is slightly out of date, I'll be putting up a new video in some time. Oh yeah, that the removal of certain functions means that the mod doesn't really work in l4d2 right now. Sorry. :(

Man, make zombies tougher "z_health" and slower "z_speed". And lower teh limit of'em: "z_common_limit". I'm not sure about the last cmd though. You should probably check it out. You can do some really amazin stuff with these cmds. Good luck, mate.

In my opinion, I think you should hide the primary weapons, like in spots where double pistols would be and stuff. And the Ammo would be lower. (Pump shotgun would have 48, Auto Shotgun would have 50, Uzi/SMG 200, M16 150, and Sniper 100)

Yeah, in this boomer are slower, have more health, and different range on their puke (not represented in this video) so they're like a miniboss encounter now.
