Eastern poems don't usually have formal titles, they are more usually known by their first line. In this case, that would be "shamali laalazaar baashad ba maa chi".
As to the author, I don't know who it is. To me this sounds like a folk poem in the "do bayti" (twin couplets) form.

His translation is a bit off. The first verse of the poem goes like this:
If the Northern Plains are blooming with tulips, what does it matter to me?
If its bitterly cold winter has turned to spring, what does it matter to me?
My nights are spent weeping and my days pass in tribulation
If the Beloved is left there waiting, what does it matter to me?
