It's actually very simple, simply develop the top of the voice at the expense of what is called the "chest voice" that EVERYBODY has. Very high tenors are rare who still retain the lower part of the voice. Notice there is hardly any vibration, actually closer to a white tone that was NOT cultivated over the 400 or so years of 'bel canto'. Castrati still retained the possibility of natural vibration. 'Counter-tenor' in this style is a middle to late 20th century phenomenon.

This rendition (imho) is comparable in beauty to the rendition by the famous tenor and former treble Peter Schreier. Not many counter-tenors have the quality and smoothness this counter-tenor has and in my opinion his voice is as unique (in a different way) to the first counter-tenor I ever heard way back in the early 60s and that would be Russell Oberlin.

There used to be a type of singer known as a "castrato." They underwent surgery before a certain age to keep their voices from changing.

@Livisfunatic falsetto is an approach, not really anything biological. You can also falsette.

today there is no castration anymore... okay maybe in india i dunno...but normally they used the head voice or the falsetto.... u can go and ask wiki... but this is definetely possible... e.g. i can sing around 3 octaves... from the E to the f'

Nowadays, like c1wang says, it's a matter of limits set by inborn factors and external ones, like vocal training.
