This is not my example, but it is a good one nonetheless.
4 ppl voted Blue, 1 voted Red, 1 voted Red & 3 voted Red; Red wins majority at 5/9. Introduce IRV & Green. 4 ppl vote Blue 1st & Red 2nd. 1 voted Red 1st & Blue 2nd. 1 voted Red 1st & Green 2nd. 3 voted Green 1st & Red 2nd.
