Clone your Enemies!
Step 1: Go to San Gimignano and climb a tower with guard in it
Step 3: Get his attention
Step 4: Go into hiding and wait for Incognito
Step 5: REPEAT STEPS 3-4

Poison a guard then assassinate him and you get the "doctor" achievement/trophy. Easy as.
