This scene is epic because the doc starts to understand how you start to become unethically in the field dealing with the players as James Woods tried to tell him. Woods tried to tell him the game is harder than you think to stay true to the field but in this scene hes giving him the shot even though he doesn't need it.

Cortisone does nothing with pain and is not like an epidural. Cortisone is a corticosteroid that players can use to reduce inflammation caused by injuries to restore function and flexibility to a joint or something similar. That's what doesn't really make sense in this movie. Shark asks for it likes its some pain killer he's addicted to but it's not addictive nor does it do anything for pain.
