I saw the singer do an interview where he explained that the song is about (last chance to avoid spoiler)
Cancer. he was diagnosed with the big C and that is the monster under his bed. The angel with her hand on his head refers to the loving support his wife gave him while he was sick.
The rest of the song including the "all you sinners" stuff is a general warning not to waste your life because you can get some shocking bad news (like a cancer diagnosis) at any second.
