Enter the operating room and see first-hand how nose surgery is performed and learn about each specific procedure with Beverly Hills Rhinoplasty Specialist Dr. Paul S. Nassif, a distinguished facial plastic and reconstructive surgeon internationally known for his innovative surgical techniques and expertise in revision & ethnic rhinoplasty.

In this video you'll discover how a conservative cephalic resection of lateral cartilages alone can be effective in addressing a mildly bulbous tip and promote tip refinement.

In this technique, a resection of a portion of the tip cartilage along the upper edge is undertaken. Known by surgeons as cephalic resection, this technique is an-other important and useful method for refining the bulbous tip.

OPERATIVE TECHNIQUE:
The columellar incision was marked with a surgical marker. The nose was infiltrated with a total of 17 cc of 1% lidocaine with 1:100,000 Epinephrine. Using a 15 blade, bilateral marginal incisions connected to a transcolumellar broken line incision were made and the soft tissue (3+ mucoperichondrium) was then elevated off the lower lateral cartilages and nasal dorsum and saved. The cephalic margin of the lower lateral cartilages was resected leaving approximately 7.0 mm in width (native width was 9 mm). The vestibular mucosa was dissected from the lateral, intermediate and medial cartilages allowing for medialization of the lateral crura (lateral crural steal).

  • Beautiful work! I have been looking for someone knows how to do african american rhinoplasty while maintaining the integrity of the nose. Her nose is similar to mine and you did just that! Do you have any recommendations for plastic surgeons in the Northwest Arkansas / Dallas / St. Louis area?

  • @lilangelz702808

    Hello,

I would recommend Dr. Greg Branham in St. Louis and Dr. Sam Lam in Dallas. They are both excellent.

  • I have a question. Now i'm sure that they CAN'T wake up.. but if they did at this point .. what would happen.

  • @WarriorInternationa1 Hello,

Anesthesia awareness is an extremely rare occurrence, and it is very unlikely to happen during a plastic surgery procedure. In the hands of a highly skilled board certified anesthesiologist, it is even less likely to occur. Thank you for the question.

  • Everyone: It's better not to watch the videos because they freak most people out. Just go ahead and trust that a reputable, experienced surgeon knows what he's doing, and is very well trained, and he/she has done this operation perhaps hundreds of times. If you want suregry, don't wait. Go and do it. You will then be happy.

  • Susie, and everyone else: Get the surgery done. There is no actual pain after the surgery. There is swelling and bruising, but this will mostly be gone in 1-3 weeks. I know, I had 5 surgeries. (my case is unique and complicated.)

  • The tip of my nose is asymmetrical.

  • OMLORD.....

  • I am currently researching and I would love to get this procedure done. I trust Dr. Nassif would do a perfect job on my nose and I am sure he would have a field day working on it. I am just scared of the pain! Anyone out there have the Tip Cartilage and nostril base reduction? What kind of pain did you feel? How long did it take to heal?

    Cheers & Thank you!

  • that make me Vomiting
