This game is Crazy Otto, which is Ms. Pac-Man's prototype, actually an enhancement kit for Pac-Man developed by GCC (General Computer Corporation), a company that made games for Atari. Eventually after a law-suit and-a-half, Midway ended up releasing the game as Ms. Pac-Man, the first sequel Pac game.
