Part 4: Preventing confined space accidents
This four part video is designed for municipal workers and illustrates the basic rules for working in confined spaces. Oxygen deficiency and other potential dangers are covered. In addition, the health effects and physical characteristics of toxic gases including carbon monoxide, hydrogen sulphide, and methane are examined.
These videos are somewhat-out dated. WCB has revised many procedures in the past 5 years and have become even more stringent.

I didn't like the way the worker opened the hatch and immediately looked into the hole, then performed a test.
If Methane or another light gas was present, the worker may have been affected.
The worker should open the hatch while keeping away from the opening or wear a respirator
This also should apply for looking into the hole to check your workmate in the hole, a means of communication (wired intercom or radio) should be used.
In the UK we have to wear a harness and a monitor all the time
