Actually, that change came from ST:TNG in 1987.

why does CBS have this as the third episode? I believe it was the first after the pilot?

CBS calls it the third episode because it was the third aired. But you're right: it was the second ever made, and was actually the second pilot. That's why the uniforms and bridge are different. It doesn't have McCoy yet, either.
