dude... when using a driver or woods, you place the ball on your left foot. Ball in the middle can cause slices and low ball flight.

Your head is definitely dipping down at the top of your backswing and on the downswing. It's hard to tell because of the view but that could be affecting your spine angle a.k.a. your posture. A little bit of back and forth sway is absolutely fine, but you'll want to be careful about the up and down movement.

3 wood swing is much better.. keep that head still
