The Great Old Ones are Cthulhu and his brethren, imprisoned in various locations on Earth. The Elder Gods are the ones who locked them there. The Outer Gods are ruled by Azathoth, and tend to be thought of more as cosmic forces than entities; some of them, like Nyarlathotep, Shub-Niggurath and Yog-Sothoth, often associate with the Old Ones.
