During Ottoman times didn't "Kostantiniyye" refer to the metro area as a whole, while "Istanbul" referred only to the city centre (presumably the area inside the Byzantine walls)?
AFAIK pre-modern Christian cities were usually entirely enclosed by their walls, while Islamic cities had significant areas outside the walls (usually commercial areas).

anyway the city took the name istampul only after cemal attaturk let the way for the turkification of all the ottoman country,-what is left over at least.
