What's funny is that Youtube is asking if I want to promote this video (that includes making money off of it). That is proof that they don't pay attention!
The below description is NOT very helpful. Reading it is like solving a puzzle, where you figure out which pieces fit and which do not belong in your case.
Detailed description on how to get your "permanently disabled account" back up and running with nothing deleted.
For those who have three or more copyrights: "send an email to Youtube asking why they deleted/suspended your account. Then you go to the help page and send in a counter argument" (wepeeler).
I need to know if the link below doesn't work for you because it definitely worked for me and I'm grateful to HKurk on http://help.youtube.com/group/youtube-feedback/browse_thread/thread/12e8f73bc... that provided the website with the link below.

NOTE: You no longer have to fill out a survey. The one mentioned in this video no longer exists. Just type in your username, email, and, if you want, a reason for appeal. Go to the link below to do this.

Below is an example of a specific email. An email like this means that you have dispute at least one of the copyright claims to reinstate your account.
"Hi mioune,
Thanks for your email. We received multiple notifications from various
content owners regarding unauthorised content that has been uploaded by
your account(s). When we're notified that a particular video uploaded to
our site infringes another's copyright, we remove the material, as the law
requires. US Federal law requires that we terminate accounts when they are
found to repeatedly infringe copyright. Because you have had other videos
rejected in the past, we are unable to reinstate your account. Users with
suspended or terminated accounts are prohibited from creating new accounts
or accessing YouTube's community. You will need to resolve at least one of
these penalties before your account can be reinstated. You may be able to
resolve at least one of the following video removals by filing a
counter-notification.
The following videos have been removed from your account:
Penalty 1:
"Spongebob Squarepants - Squeaky Boots (french)" formerly at
http://www.youtube.com/watch?v=OaC5fkWEsLg
Removed due to a copyright claim by Viacom on 11/02/2006
Penalty 2:
"Utada Hikaru & Disney Girls - First Love" formerly at
http://www.youtube.com/watch?v=vE50lCc1pxY
Removed due to a copyright claim by JASRAC on 01/25/2007
Penalty 3:
"Spongebob Squarepants Theme (french)" formerly at
http://www.youtube.com/watch?v=MXSO6RJF7Ko
Removed due to a copyright claim by Viacom International Inc. on
02/10/2007
IMPORTANT: If you feel that a content owner has misidentified your content
as infringing, you may be able to resolve one or more of these penalties
by filing a counter-notification. For more information, please visit our
Help Center article about counter-notifications at

@3434343434utubeguy Hey, when your account was suspended, had all your comments you've ever made disappeared too? That's happened to me!
I've been waiting for 3 hours for youtube to get back to me after I'd filled in the form, does it normally take this long?
terminatedaccount07 1 month ago
@terminatedaccount07 I took like a day for me, but this was like 3 years ago.

@3434343434utubeguy Hey, do you know if they still bring back old accounts this way? My account was terminated in June 2009 for copyright infringement for a video game. I've been trying to get it back since. I'm gonna try to file counter claims for each of the infringing videos to see if one will work...
