He said the amount of blood it would take Sam to kill Lilith will change him forever. He would become the next creature that Dean would feel conpeal to kill. That means Sam doesn't actually have to kill Lilith, but drink enough blood so he can be strong enough to kill Lilith. He would become the creature that Casteal was talking about only if he drink just enough blood to kill Lilith.

SPOILER.Castiel takes Sam to a spiritual journey to make him realize that he can't go on drinking demon blood. Maybe Castiel does this because Dean offers to serve him and the angels for exchange. But Sam takes a full turn and the journey makes him want even more to revenge Lilith. Damn I can't wait.
