You need more spine angle at address. Kick that right hip in and slightly down so your inner right leg is pushing down.
You're also starting the swing with your hands. Try to feel the club slightly lagging away from the ball.

Right leg locks up. keep it flexed and turn into it - proper pivot. Your club plane is quite good.
