This is a patient referred in with an ACIOL and endothelial decompensation after complicated cataract surgery. A decision was made to leave the ACIOL in place and just do DSAEK rather than to replace the IOL as there was no evidence of CME or glaucoma and the implant was well positioned. Here a Busin glide is used with an MST 23ga microforceps to pull the tissue into the AC where it easily unfolds over the ACIOL. NOTE that the descemets stripping has already been performed here but is not shown in this video. That portion is done with a reverse Sinsky hook and the membrane removed in one piece with a microforceps. At the end of the video the day one post op slit lamp image is shown.
