An updated version of my original guide. This one actually shows a 5v5 guide. I noticed a handful of players complaining about my original guide... saying I wasn't any good because I was playing in low elo and a 3v3 guide isn't professional at all. Well, my score disagrees otherwise.
Personally, I prefer playing Fiddlesticks mid-lane due to his great ganking potential to gank all lanes early game. Remember that if you have a jungler, not only can your jungler gank all lanes, but you can help as well.
For Masteries I go the classic 21 in offense, 9 in utility. Picture can be found here:

For Runes I go 2x Greater Quintessence of Force, 1x Greater Quintessence of Insight, 9x Greater Mark of Insight, 9x Greater Seal of Force, and 9x Greater Glyph of Force for a combined total of:
Ability Power at level 18:
+ 60
Magic Pen:
+10

I've learned that as long as you play defensive early game (due to Fiddlesticks super squishiness), you don't have to worry about defense at all... ever. His primary concern is getting his ability power up as much as possible, as fast as possible. The small amount of Magic Pen helps as well as some people grab magic resist runes for early game. I've noticed that early game is Fiddlesticks' greatest weakness as his hard CC (Crowd Control) doesn't last too long while his late game presence is a monster (definitely one of the best in the game). As long as you do what I do in this game and concentrate on staying defensive and getting those last hits in, you should never have to worry about mana problems or ganking problems.
I've noticed a lot of players being concerned about enemy Morigans, Annies, Kassadins, etc. This is because you probably are pushing your lane too well. You need to back up some. They have great stun potential and can easily kill a Fiddlesticks, but I never have problems w/ them due to my early game harassing w/ Dark Winds. Play defensive and you shouldn't have to worry about any champion.
For order of equips:
I start off w/ an Amplifying Tomb and 1 health pot if I'm doing jungle. However, if I'm going in a lane (preferably mid-lane) I start w/ a Doran's Ring. Unlike Twisted Treeline, you can't go back and heal up your mana real fast nor is Clarity as OP as it is in TT.
Directly after you get enough gold, always, ALWAYS get the magic pen boots. I've found that the benefit of grabbing his upgraded boots outweighs the ability items early game since a slow Fiddlesticks = a dead Fiddlesticks and many of your enemies will have upgraded boots early game as well.
The 3rd equip depends on your game.
- If you're doing extremely well or the enemies over-extend a lot (like you saw in this game), I highly encourage grabbing a Majai's Soulstealer. The gank potential w/ Fiddle is way too high to pass this one up.
- If you're having problems w/ the enemy team targeting you first (which smart teams do), ALWAYS grab a Zhonya's Hourglass. Not only will it protect you during those tough times, but it'll boost your AP and allow you to do the Ult + Hourglass trollface combo.
- If you have a normal game w/ good players on both sides and it's looking like mid isn't stupid enough to push the lane, ALWAYS grab a Rabadon's Deathcap. Being a carry, you need to focus on Fiddlesticks gaining as much AP as possible as soon as possible. Your team relies on you for ganks in team fights and if you don't have enough AP, you won't do enough damage. That's why having +60 ability power @ lvl 18 is so good. The runes will help boost your Deathcap equip w/ every lvl and is so good not to pass up.
- For your 4th equip and so on it always is based upon the state of the game. Having trouble w/ CC? Banshee's Veil all the way. Need more AP? Grab a Crystal Sceptar to add the slowing passive so they can't run out of your Crowstorm as fast. Need more defense? Grab items that boost both AP & health, mag. resist, or armor while always focusing on AP as the priority.
One more thing I'd like to add. Once you get to lvl 18 you can solo Baron. Just grab the blue buff and spam your drain. W/o blue buff you run out of mana too quickly so don't try it.
I hope that this guide has helped out any newbie that wants to learn how to play Fiddlesticks as well as any advanced players who are looking for possible new ways to play that they may have not have thought about before. Furthermore, while my elo isn't too high, know that I've gone as low as 800+ elo and am slowly gaining it back again. You win some, you lose some, but if there's any champion I do know how to play professionally, it's Fiddlesticks all the way.

U need to drain him while he is feared so he cant run or stun u..
so the combo should be Q W E or E Q W..

@MsSebi1111 You only want to use Drain as a last resort to conserve mana. Anyway, since this video I always throw out E first, wait a second, then cast Q, and last my Drain.
