Hijacked bus. His dudes were already in it and the other buses had left.

Disguise. He infiltrated the hospital to get to Dent. Go watch the movie. Oh, BTW, Dent/Two-Face dies at the end and Joker goes to jail.

He had to wear one to get into the hospital.
He was posing as a nurse so he can go there with out getting caught. Thus planting the bombs.
